As we age, the connective tissue around our eyes loosens, allowing gravity to pull our eyelids down and to cause the normal fat around our eyes to bulge outward. Eyelid surgery (also called blepharoplasty) is designed to remove unsightly excess skin and to recontour the shape of the eyes to give them a younger appearance. Recontouring is the key to a successful result. For the upper lids, this most often involves removing the excess skin that is drooping down. In lower lids, recontouring mainly involves finding a solution for the bulging fat that causes our lower lids to look puffy. For years, the standard approach was simply to remove the fat pad behind the skin, and for some people this still works well. But we have found that, for many people, repositioning the fat without removal can yield a more natural-looking and beautiful rejuvenation. We select the approach that works best for a person's unique anatomy. Where does the laser come in?
The laser has made a major contribution to cosmetic eyelid surgery by making it possible to replace a traditional steel scalpel with a laser light scalpel. The advantage is reduced bruising, which in turn permits a faster recovery period. It is not uncommon for our laser cosmetic eyelid surgery patients to be back to work in just a few days. We also use the laser for its ability to tighten and smooth skin. Switching from the "incisional" mode to the "pulsed" or resurfacing mode, we can combine skin resurfacing with blepharoplasty to reduce the skin wrinkles (such as crow's feet) that the eyelid surgery alone cannot address. Speaking of combining procedures, the browlift is another procedure that is used in conjunction with eyelid surgery for some patients. A browlift is indicated when sagging eyelids are made worse by the loss of elasticity of muscles and skin of the forehead and brow.
